Application form for an absentee ballot
Absentee balloting at congregational meetings is allowed in the bylaws only for an eligible voting member who is unable to attend in person due to a disability such as being hospitalized, confined to home by doctor’s orders, etc.
The absentee ballot permits voting for:
- A nominee for office.
- Any resolution that is not amended during the meeting. If a resolution is amended, the absentee vote, which was for the original form of the resolution, will not be counted. The rest of the ballot is unaffected.
Please provide the following information:
This form must be submitted at least one week prior to the meeting for which you are requesting a ballot.
